2023 Awards Nominations are closed. Awards will be presented at the CSDA House of Delegates luncheon and nominations will open in November 2023.

Award Categories Are:

 

Fones Medal

The Fones Medal is presented annually to an individual in recognition of outstanding
contributions, achievement and dedication to the science and/or practice of dentistry
or it may be presented to an individual for outstanding achievement in the interest of
humanity.

Horace Hayden Award

The Horace Hayden Award is presented to an individual or organization in the field
of communication for contributions to the interest of dentistry and dental health.

Sal Squatrito Legislative Achievement Award

The Sal Squatrito Legislative Achievement Award is presented to an individual or
organization that has significantly impacted legislation in the best interests of the
public or CSDA.

CSDA Presidential Award

Presented to a CSDA member, or any dentist, group, or other contributor that has
enhanced the image and professionalism of dentistry. Nominations for the
Presidential Award will be at the discretion of the CSDA president.

Oral Health Champion Award

This award may be given to any outside organization that has shown or produced a
major initiative for improving the oral health of the citizens of Connecticut. This
award can be recommended by any member of the organization, the Executive
Director, or Board of Governors.

CSDA Distinguished Service Award

The CSDA Distinguished Service Award is presented to a Life member who has
shown exemplary service to the CSDA over his/ her dental career. Requirements are:

  1. Life Membership
  2. 25 years or more of membership in the tripartite system
  3. Approval of the Board of Governors

Task Force/Special Projects

Task Force/Special Projects(s) will be presented to the CSDA member who has
shown exemplary service over the year on special ad hoc projects or Task Forces. The
award can be recommended by any member of the organization, the Executive
Director, or Board of Governors.

CSDA Staff Recognition Award

CSDA Staff Recognition Award will be presented to the CSDA staff member who
has shown exemplary service over the year. This award can be recommended by any
member of the organization or the Executive Director.

Etherington Award

The Etherington Award is an award presented by the Massachusetts Dental Society during the Yankee Dental Congress. The purpose of this award is “to recognize dentists of New England who have dedicated much of their professional lives to the high ideals and objectives of organized dentistry.”

The CSDA nominates a member for this award when a worthy candidate is identified.

Rising Star Dentist Award

Eligibility: Nominees who satisfy the following criteria will be considered -

  • Residents and New Dentists are eligible for this award if they are a member in good standing of the Connecticut State Dental Association and graduated from dental school in the past 10 years; or
  • Dental Students are eligible for this award if they are a member in good standing of the UConn Chapter of the American Student Dental Association.

Submission: Any member in good standing of the Connecticut State Dental Association may nominate an eligible candidate for this award. Members can also nominate more than one candidate for consideration. However, the only limitation is that self-nominations are not permitted for this award. 

Selection: The Awards Committee may select up to five eligible nominees each year to receive this award.  In making these selections the Awards Committee should look for nominees who have distinguished themselves through one or more of the following activities:

  • Produced exemplary work;
  • Actively engaged in advocacy, philanthropy or community service;
  • Recognized for their practice excellence, entrepreneurial skills, or ethical behavior;
  • Made a significant contribution in the field of science; or
  • Held a leadership position in the ASDA Chapter of their dental school, in their local dental society, the CSDA, or ADA.
